What is Dextromethorphan Hydrobromide?   

Dextromethorphan is a pharmaceutical drug which is much popular as an over-the-counter or OTC medicine to control cold and cough, falling under the category of antitussives.

It is in the morphinan class of medications with sedative, dissociative, and stimulant properties at lower levels, and does not have any significant affinity towards the mu-opioid receptor activity typical of morphinan compounds. 

 

How does Dextromethorphan Hydrobromide work? 

Dextromethorphan is a prodrug of dextrorphan, which is the actual mediator of most of its dissociative effects by acting as a more potent NMDA receptor antagonist than dextromethorphan itself. 

In other words, Dextromethorphan is an agonist of NMDA and sigma-1 receptors. It is also an antagonist of α3/β4 nicotinic receptors. However, the mechanism by which dextromethorphan's receptor agonism and antagonism translates to a clinical effect is not yet well-defined. 

Being a type of CNS drug, acts directly on the central nervous system, elevates the threshold for coughing, without inhibiting ciliary activity. Dextromethorphan is rapidly absorbed from the gastrointestinal tract and converted into the active metabolite dextrorphan in the liver by the cytochrome P450 enzyme CYP2D6.
